I think a couple of other things may be interesting:
1. How do you like to get Jenkins support (similar to "How do you find plugins"), i.e. via colleagues, StackOverflow, IRC, mailing list, support contract with X? 2. A free text comment field at the end would probably be interesting, for any free-flowing thought from users.
